International Competitiveness The ability of an economys firms to compete in international markets and, thereby, sustain increases in national output and income. L2. Monetary policies can be used to promote economic growth, Economic (this stability reduces uncertainty, promotes business, consumer confidence and investment) and International Competitiveness. This causes an ? in AD, which can be good for an economy. It also depends on when interest rates are changed as to what else is going on in the economy at that time, for example if there is a fiscal policy causing income tax to ? at the same time as a ?in interest rates the affects of the Monetary Policy may be cancelled out by the fiscal policy. It depends on Central Bank bringing creditability to the target as the central bank has to build up a reputation for meeting targets. This can lead to low economic growth being traded off for low inflation in the short run, but not the long run, which is what is needed for an economies economic growth to be sustainable! The Central bank must be good at forecasting inflation, as the Monetary Policy works with time lags, there can sometimes be a two year delay! So the Central bank will have to set today the interest rate to affect the rate of inflation it expects in two years time! For example Inflation targeting has to be guided by forecasts of inflation and all macroeconomic variables that affect inflation. It also costs a great deal to employ people who have the ability to forecast inflation well which could cost a lot to employ someone capable of doing this, this means that it ? costs, which means the possibility of an opportunity cost involved as that money could have been spent on something else for example new hospitals. There can also always be unforeseen circumstances such as unexpected recessions and natural disasters such as the tsunami, this affects the Central Banks ability to deliver economic stability and economic growth as they do not know if they may need to be doing other policies to help these unexpected situations, as they may only be able to do so when the economies conditions are stable. Sickle Cell Anemia Case Study A 20-year-old Africa- America woman visits her physical complaining of episodes of extreme pain and discomfort in her legs and lower back. She has been experiencing these recurrent episodes, accompanied by extreme fatigue, since she was a child. On physical examination, she appears jaundiced and has a hematocrit of 23% and a hemoglobin level of 7g/dL. She reports she has family members who experienced the same symptom. Sickle cell anemia (sickle cell disease) is a disorder of the blood caused by an inherited abnormal hemoglobin (an oxygen-carrying protein within the red blood cells). The abnormal hemoglobin causes distorted (sickled) red blood cells. The sickled red blood cells are fragile and prone to rupture. When the number of red blood cells decreases from rupture (hemolysis), anemia is the result. This condition is referred to as sickle cell anemia. The irregular sickled cells can also block blood vessels causing tissue and organ damage and pain. Sickle cell anemia is one of the most common inherited blood anemias. The disease primarily affects Africans and African Americans. It is estimated that in the United States, some 50,000 African Americans are afflicted with the most severe form of sickle cell anemia. Overall, current estimates are that one in 1,875 U.S. African American is affected with sickle cell anemia. Sickle cell anemia is caused by a point mutation in the ÃŽà ²-globin chain of haemoglobin, causing the hydrophilic amino acid glutamic acid to be replaced with the hydrophobic amino acid valine at the sixth position. The ÃŽà ²-globin gene is found on the short arm of chromosome 11. The association of two wild-type ÃŽà ±-globin subunits with two mutant ÃŽà ²-globin subunits forms haemoglobin S (HbS). Under low-oxygen conditions (being at high altitude, for example), the absence of a polar amino acid at position six of the ÃŽà ²-globin chain promotes the non-covalent polymerisation (aggregation) of hae moglobin, which distorts red blood cells into a sickle shape and decreases their elasticity. The loss of red blood cell elasticity is central to the pathophysiology of sickle-cell disease. Normal red blood cells are quite elastic, which allows the cells to deform to pass through capillaries. In sickle-cell disease, low-oxygen tension promotes red blood cell sickling and repeated episodes of sickling damage the cell membrane and decrease the cells elasticity. These cells fail to return to normal shape when normal oxygen tension is restored. As a consequence, these rigid blood cells are unable to deform as they pass through narrow capillaries, leading to vessel occlusion and ischaemia. The actual anaemia of the illness is caused by haemolysis, the destruction of the red cells inside the spleen, because of their misshape. Although the bone marrow attempts to compensate by creating new red cells, it does not match the rate of destruction.[17] Healthy red blood cells typically live 90-120 days, but sickle cells only survive 10-20 days.[18] Normally, humans have Haemoglobin A, which consists of two alpha and two beta chains, Haemoglobin A2, which consists of two alpha and two delta chains and Haemoglobin F, consisting of two alpha and two gamma chains in their bodies. Of these, Haemoglobin A makes up around 96-97% of the normal haemoglobin in humans. In normal Haemoglobin A, glutamic acid is on the 6th position of the beta chain, while in sickle-cell disease, this glutamic acid is replaced by valine leading to the formation of sickle cells. This happens due to a one point mutation. This leads to polymerization of the two beta chains and therefore their appearance as puzzle pieces (or lock and key); which means they fit into each other forming a longitudinal polymer that would lead to the cell becoming deformed and very rigid leading to vessel occlusion. This process of polymerization can be activated by infections, hypoxia, acidosis, physical exercise, vasoocclusion due to cold as well as hypertonic dehydration. Diagnosis Sickle cell anemia is diagnosed through blood test, testing for hemoglobin S (the defective form of hemoglobin descriptive of the disease), the presence of other abnormal hemoglobin variants, evaluating status and number of erythrocytes, and/or determination of one of more altered hemoglobin gene copies. In the United States, this blood test is part of routine screening for newborns done in the hospital. However, older children and adults can be tested also. In adults, the blood sample is extracted from a vein in the arm. In young children and babies, blood is taken from a finger or heel. The testing itself is typically performed on a smear of blood utilizing a special low-oxygen preparation, known as sickle prep. Other prep tests can be utilized, including but not limited to solubility tests. [4,6] Another screening testing is the Hb S solubility test. In this procedure, a chemical is added to the blood sample which reduces the amount of its oxygen carrying capacity. In individuals carrying even one sickle gene, some hemoglobin S will be present. The reduced amount of oxygen will cause S-related polymers to form and affected erythrocytes will sickle. This test, in essence, detects for the presence of Hb S alone. However, this exam should not be performed on infants until age six months, as babies with sickle cell will not produce significant amounts of Hb S until several months after birth. [6] To confirm the diagnosis, DNA analysis can be utilized. This exam is used to detect alterations and mutations in the genes producing hemoglobin components. DNA analysis reveals one copy or two copies of the hemoglobin S gene, or copies of different hemoglobin variants. DNA analysis can be performed on the developing fetus in fourteen to sixteen weeks gestations via amniocentesis or through chorionic villus sampling.[6] Treatment Treatment of sickle cell anemia is done by blocking the red blood cells from stacking together. the health professional maintenance helps the patients to begin with early diagnosis of the disorder, preferably during the newborn period. Penicillin prophylaxis, vaccination against pneumococcus bacteria, and folic acid supplementation is standard. However, since erythrocytes contain iron and the body does not possess a natural process for its elimination, patients can accumulate iron in the blood. Thus, possible iron toxicity must be closely screened and methods to remove excess must be executed. Excess iron is removed artificially through administration of the drug Deferasirox (Exjade Ãâà ®) orally in patients two years of age and older. If this is not checked, the excessive iron can accumulate in the heart, liver, and various other organs causing organ damage. [3] Other treatments for this disease include finding a substance that prevents erythrocytes from sickling without producing deleterious effects to other body areas. The medication hydroxyurea has been found to reduce the frequency and severity of pain, acute chest syndrome, and decrease the need for blood transfusions in adult patients. Droxia Ãâà ® (prescription name brand formulation of hydroxyurea) was approved by the Food and Drug Administration in 1998 and is currently available for adult patients. Studies are currently being conducted to determine the proper dosage in pediatric patients. However, there is concern in this medication that chronic usage may facilitate tumor growth or leukemia in certain individuals. [5] Other pharmacological therapies include antibiotics and pain relievers. Regarding antibiotics and aforementioned earlier, children benefit from penicillin at age two months and usually continue medication until age five. Prophylactic antibiotic treatment helps in preventing pneumonia. For adults, prophylactic antibiotic treatment can aide in fighting certain infections that they would normally fight provided they had normal erythrocytes. [2,3,5] Non-pharmacotherapy treatments are bone marrow transplant and supplemental oxygen therapy. Bone marrow transplant procedure offers the only potential cure for sickle cell anemia. Replacing the system with unaffected bone marrow aids the body in producing normocytic erythrocytes. However, finding a donor provides difficult even with the advent of registries. Also, the extraction of bone marrow possesses serious risk, including death. After the procedure, a mandatory lengthy hospital stay is required. In the hospital and upon leaving, the patient will be administered medications to help prevent rejection of the donated marrow. [3] Since sickle cell anemia is caused by a defective gene, researchers speculate that insertion of a normal gene into bone marrow of people with sickle cell anemia will result in the production of normal hemoglobin. Another gene therapy possibility is turning off the defective gene, while reactivating another gene responsible for production of fetal hemoglobin (a type of hemoglobin found in newborns) that prevent sickle cells from forming. [3] Butyric acid, normally utilized as a food additive, may increase the amount of fetal hemoglobin in the blood in some patients. [3] Clotrimazole, the over-the-counter antifungal medication may help prevent loss of water from erythrocytes, possibly reducing the number of sickle cells formed. [3] Nitric oxide is decreased in sickle cell anemia, a gas that normal causing vasodilatation. Administration of this agent would prevent the sticking of sickled cells to one another.[3] Nicosan, an herbal treatment in early trials in the United States, is currently being used to prevent sickle crisis in Nigeria (West Africa). Animals In The Eyes of The Dragon Although it may seem quite absurd, the role of animals in The Eyes of the Dragon is a very supreme and dignified part of the novel. Through the development of the novel, the animal kingdom roars novel from insects to dogs and they all play their own, individual roles. The great black and white Anduan Husky named Frisky who is, out of all of her dogs, Naomi's favorite, leads Ben Staad and Naomi directly to the whereabouts of Dennis, Peter's butler. Flagg's animals are symbols of his plans for the downfall of Delain. The dragon that is killed by King Roland may be considered the most the memorable of all. In this archetypal tale, Thomas sees Flagg murder Thomas's father through the eyes of the dragon. In the end of the novel, though, all of the animals' roles fall into place. Frisky, Naomi's companion, is a strong-willed, over-confident, Anduan Husky who may have been the "greatest tracking dog that ever lived." This dog sums up the meaning of a man's best friend. Frisky, who can track a three-day-old scent in the middle winter, is the reason the story takes place as it does. Just as arson dogs help pinpoint the location of substances used to start fires, Frisky uses her keen sense of smell to pinpoint exactly where Dennis, son of Brandon, has journeyed to from Peyna's farmhouse. Dennis's mission is to go back to the castle where Thomas the Tax-Bringer and Flagg, the king's magician, live and are at high power. Peyna, who has just given up his Judge-General's bench, has a feeling that there is some reason why Peter has asked for the Royal Napkins and his mother's dollhouse. Dennis is to find this reason by sending a letter to Peter, who is imprisoned at the time. Dennis journeys from Peyna's farm in the Inner Baronies back to the fearful castle with nothing except for the risks of getting killed. Five days later, with not a soul knowing the whereabouts of Dennis, Naomi and Ben Staad (Peter's best friend) come upon the deserted farm. Naomi's dogs have been pulling the sleds for thirty miles all the way from a snowy hollow in which they were camping. The two now feel helpless. Fast Food Currently, most of the people depend on fast food which is easily accessible rather than cooking food at home which is a time consuming task. Fast food is the most popular food in America. Fast food is a quick way of filling up your stomach but it is followed by many unknown and known disadvantages. In my opinion there is only one reason why anyone should rely on fast food couple of times and that is time saving. It saves time. Since most cowboys and ranchers have gone out of business, they have become irreverent in todayââ¬â¢s culture. Later, the Reagan administration allowed the top four meat packing firms to merge and they took over the cattle markets. Corporate domination affected family farms by forcing them to work for them or go out of business. Self-reliance is still a viable goal for Americans, but has become very difficult because of big corporations. The fast food industry makes chicken farmers work for them or they would get no business. In farm culture, the land is a tangible connection to the past, meant to be handed down and not sold. Chapter 7 The demands of the fast food industry changed towns by making is so almost everyone ends up eating fast food because the industries are aloud to put their restaurants wherever they want and advertise as much as they want until the townââ¬â¢s economy runs on the fast food restaurants. Chapter 8 Meatpacking is dangerous because of the machines and rarely cleaned cutting tools. Since a meatpacking managerââ¬â¢s bonus is based in part on injury rate, many injuries go unreported and the worker is either given an easer job to takes time off to recover. Chapter 9 The meat packing and meat processing industry has been a spreader of disease because the animals are not screened well enough or sick workers spreading disease on the animals. After reading about the pathogens in hamburger meat, I am concerned about food poisoning in fast food. E. coli is not common in restaurant food, but is likely to be in hamburger meat. Chapter 10 Many Americans are obese because of lack of information and improper food laws. Fast food probably plays a big part in obesity because it provides unhealthy food to almost every where. Americans are probably more obese than other countries because fast food started here. Fast food companies increase the size of their meals to effect the calorie count look better for the per serving part. This effects American health because it is misguiding and you eat more than you thought you were going to. People in other countries do not want fast food because they have seen its effect on America. Epilogue The free market Schosser talks about leaves workers unprotected with little interference from the government. In the quote, Schosser is referring to the free market. I agree to what Schlosser says on 216. I believe the government needs to work harder to protect both the workers and consumers of fast food. At the end of the Epilogue, the author remedies his criticisms with the fact that it is a persons choice to eat want. Afterword Mad Cow disease is a disease that slowly destroys the brain and can be spread through hamburger meat. It can be controlled by feeding cows grass instead of corn and hormones, and inspecting the meat better. Cattle get infected by it because they stand in the dung of an effected cattle. It is a very serous threat to humans. Fast food Fast Food Currently, most of the people depend on fast food which is easily accessible rather than cooking food at home which is a time consuming task. Fast food is the most popular food in America. Does fast food proves to be a healthy option for a daily purpose? Is fast food healthy? Being an argumentative topic this term ââ¬Å"fast foodâ⬠comes up with many questions. Fast food is a quick way of filling up your stomach but it is followed by many unknown and known disadvantages. As a peek in the ââ¬Å"Consumers reports on fast food: four big names loseâ⬠from the book ââ¬Å"TheNorton Field Guide to Writingâ⬠, the reporter mentions that â⬠Next time you have a craving for fast food, think twice before slowing down for Burger King, KFC, McDonald's, or Taco Bellâ⬠. The fast food restaurants the reporter mentioned here, are the most famous and renowned places, where the consumer is attracted to eat due to food and low prices. In my opinion there is only one reason why anyone should rely on fast food couple of times and that is time saving. It saves time. But there are many other reasons as surveyed by the reporters for people to be attracted towards fast food.One major reason is the low prices. As also mentioned in the research named ââ¬Å"Fast foodsâ⬠from the ââ¬Å"Black bookâ⬠by ââ¬Å"Emanuel Goldman- Alfred L. Simonâ⬠, that, ââ¬Å"McDonald's might not be ready with the problems with its chickenâ⬠. But, if we see as a whole the prices of McDonald's are very low which hides the fact that there have been many issues with the chicken it use. People are attracted to McDonald's even today for their meals. There are many problems caused due to the regular intake of fast foods. Some serious problems are Obesity, weight gain, type two diabetes, coronary artery disease and PCOD.Obesity is one of the most common and erious problems found in Americans. This problem is a clear and direct result of the lifestyle in Ameri ca. The habit of substituting meals with the fast food gives a birth to obesity in an individual. In the Journal, ââ¬Å"Fast Food: Unfriendly and Unhealthy' from ââ¬Å"International Journal of Obesity', the author S Stender, J Dyerberg, and A Astrup has given the example of a documentary film ââ¬Ësuper-size me', where the character Mr. Spurlock ate McDonald's food thrice a day for 30 days and gained 11 kg.This film raises a question that whether fast food poses a special health risk. This is a very mportant question to be answered for all the fast food lovers. The other problems like PCOD and diabetes are a result of fats food intake. The Polycystic ovarian disease found in most of the women these days is caused due to the weight gain. This weight gain is a result of our food habits, like consumption of fast food on a regular basis. Another reason for the increased craving for fast food among the kids is the fast food ads which are made graphically innovative so as to attract t he kids.The recent study shown by ââ¬Å"The Washington Postâ⬠in their program, ââ¬Å"Trying to Cut Back on Fast Food Ads for kidsâ⬠ââ¬Ë mentioned that the obesity in kids of age group 6-11 has gone down because some fast food chains have cut down in their advertising. This statement shows that the advertisements and obesity in kids are directly proportional to each other. This is a strategic advertisement plan for the fast food agencies to make profit. They invest more on advertisements targeting the kids as their main audience and are women.Young women are facing many problems due to the intake of fast foods. In a Journal ââ¬Å"Dieting Behaviors of Young Women Post-College Graduationâ⬠, the uthors ââ¬Å"Soliah, LuAnn Walter, Janele Antosh and Deeannaâ⬠, had conducted a survey that included questions about the eating habits of women and their psychological affinity for food. The conclusion of the survey was not shocking but very obvious that three groups of women appeared to be at an increased risk of unhealthy eating and poor food selection decisions.The unhealthy food here is the fast food. Fast food is the most selling food worldwide. There are many reasons one cannot avoid the fast food. Good taste is one of the biggest reasons. Low prices are another reason which attracts the consumer. Fast food has become the regular meal of people. Whenever I visit any food chain I can see a bunch of people spending their valuable money on something which could be harmful for their health. The conclusion, henceforth, is very clear that fast food is not healthy in any terms.Fast food consumed results in many problems which are ignored by the fast food consumers. America faces most of the consequences of eating fast food on a regular basis. Some problems are very serious like obesity and diabetes which are a direct result of fast food consumption. It is still not late for the people to put a light on the articles and surveys conducted n the fas t food being a disaster and learn a lesson through their results. Fast food industries should be responsible enough to advertise less or make their food healthy instead of cheesy and greasy.An individual should keep a track on their diet decisions rationing them with their daily intake fast food and other diet. The comparison would be very useful for an individual to realize how to make their health and wealth lives longer and strong.

How play activities are used to support the development of speech Essay
Play is an important part of developing language and young children learn through play. There are some very fun activities and games that you can play with children to support their language development, these may include; Role-play: Having a role-play area in your setting can be fun for the children and help them communicate with other children. For example, in the role-play area in our setting it was a cafà ©, one child pretended to be the assistant while one child pretended to be the customer wanting to order some food. There was lots of conversations happening and gave me a chance to observe how well the children communicated. Nursery songs and Rhymes/Song and Rhyme bag: This encourages children to listen, sing and communicate. With a song and rhyme bag, each child takes it turns to pick an object out of the bag and then encourage them to sing the song for that item. For example a bus for ââ¬Ëwheels on the busââ¬â¢, a doll for ââ¬ËMiss Polly had a dollyââ¬â¢, a spider for ââ¬Ëincy wincey spiderââ¬â¢. Books: Sharing story books with adults is considered to be one of the most important ways of developing childrenââ¬â¢s spoken and written language. Books that use repetition are most effective. Picture books with no words in are also good to encourage the children to make up and tell you their own stories according to whatââ¬â¢s happening in the pictures.
